Senior Sub Group

The St. James Town Seniors Committee is part of the St. James Town Service Provider Network. It is represented by a group of service providers working in collaboration in St James Town for community members. The purpose of the committee is to identify and support seniors 55 years plus living in the community.

Objectives

  • Identify needs, assets, gaps and trends related to Senior’s in the St. James Town community
  • Collaborate and coordinate with organizations and community members supporting seniors in St. James Town
  • To host, support and participate in Senior’s related activities and events such as the Seniors Forum every June, educational workshops as well as the St. James Town Festival

Health Access Working Group

The Health Access Working Group (HAWG) is a group of 12 health and social service organizations, who are all members of the St James Town Service Providers’ Network (SJTSPN) and form the Executive Committee of the Health Access St. James Town (HASJT) initiative. Sherbourne Health Centre is the lead organization.

Objectives

HASJT seeks to establish a neighborhood based client-focused model of integrated, collaborative care through inter-sectoral service coordination, service enhancement and community development. It aims to connect unattached and disconnected residents to health and social services.   By improving the overall health of a high need, disadvantaged population HASJT also serves as a health promotion model in creating equitable access to holistic care.

Youth Network

The St. James Town Youth Network collaborates, advocates and coordinates stakeholders inside and outside the network that address the needs of youth in the community. The member of the network provides multiple servers to youth ages 12 to 29.

Objectives

  • Identify the needs, assets, gaps and trends in St. James Town.
  • Collaborate and coordinate with required youth focused organizations to fulfill the needs of the community by jointly identifying resources
  • To develop subcommittees when necessary to implement an action plan

Outreach & Engagement Sub-Committee

As a subcommittee of SJT SPN, Outreach and Engagement committee will support and take a lead if necessary in the outreach initiatives of the network agencies. This support would be in the form of strategic directions to plan an outreach or take up the responsibility of outreach and creating a response pipeline towards programs and services. This committee is committed to long term community engagement towards a sustainable social development model. This initiative will be an ongoing cumulative process enabling relationships and trust between residents and strengthen the initiatives of SPN organizations.

Objectives

  • Identify the service gaps:- Prepare a SJT specific need assessment report from existing studies/literatures. Engage new/existing service providers based on identified service gaps.
  • Create and implement a community relevant outreach strategy to for promoting various programs and services offered by SPN agencies.
  • Create and implement a locally responsive engagement strategy to raise critical consciousness, recruit members, program inputs and gain resources for advocacy and community led initiatives.
  • Increase/re-brand the external visibility of Community Corner through outreach and engagement activities and develop synergies across community groups.
  • Develop a sustainable social infrastructure to streamline and coordinate the outreach and engagement initiatives of O & E Committee
